Driver power state failure blue screen

Event log had this share: The computer has rebooted from a bugcheck.  The bugcheck was: 0x0000009f (0x0000000000000004, 0x000000000000012c, 0xffffc48a4e51e640, 0xffffbe871ca7d8e0). A dump was saved in: C:\Windows\MEMORY.DMP. Report Id: 960e9445-c7c2-4781-a52c-1b85d1d881c3.

Have had this ongoing for awhile. All drivers are up to date to my knowledge.

Problem occurs on attempting to shutdown or restart, and it doesn't happen consistently. I can tell when it is going to occur if I launch a game and am given a black window for the app, or if Steam refuses to start despite Task Manager stating it is running in the background. Some games will refuse to start period, but usually will be the aforementioned black window

GPU was RMA'd and have swapped out for another one as another test (which it did not give a driver power state failure error, just merely crashed due to be a significantly older card). Currently trying a different CPU on suggestion ever since this crash on Monday and haven't seen anything since, but I suspect it's not related to hardware at all as the issue is incredibly sporadic. Typical CPU is AMD Ryzen 7 3700X, but don't think it may make much of a difference.

Hello,

The problem is, I would like to see if someone can interpret from the dump files to find specifically where the problem is at. The issue is so sporadic that I'm not sure if changing the power plan settings would even suggest a fix until I can pinpoint that is the root cause unless if the dump files absolutely point to that.

The minidump files were inconclusive.

Can you provide the following file which has more information on the crash:

C:\Windows\MEMORY.DMP

The memory.dmp file will be quite large, you could zip and compress the memory.dmp file with a third party application such as 7-Zip.

Can you provide the following information which may help determine any issue(s):

Windows Key + R > type msinfo32 in the "Open" box > OK > File > Save > then save as an .nfo file

Then make the resulting .nfo file available via Google Drive.

Also, can you do the following:

Windows Key + R > type eventvwr in the "Open" box > OK > expand "Custom Views" and then right-click "Administrative Events" > select "Save all events in Custom View As" and save as an .evtx file

Then make the resulting .evtx file available via Google Drive.

There was this repetitive error in the Event System Log:

Kernel-Boot - Windows failed fast startup with error status 0xC00000D4.

Can you try "turning off" Fast Startup and see if it helps:

Also, as the dump files showed a possible issue involving network drivers try updating the Intel LAN driver:

I'll try both for now. The issue is very sporadic and I don't want to declare it as fixed when it can occur again up to even a week later.

Were there any other issues that may have stuck out as the potential source? Namely nothing hardware-related? I appreciate your time on this.

Windows Error Reporting (from the System Information) showed a possible issue with the graphics - nvlddmkm.sys is the NVIDIA graphics driver and 0x141 is VIDEO_ENGINE_TIMEOUT_DETECTED

Windows Error Reporting Fault bucket LKD_0x141_Tdr:6_IMAGE_nvlddmkm.sys_Unknown

But this may have been corrected with a graphics driver update.

There was an update from NVIDIA back on the 27th, though the update did not go through until the 2nd after the blue screen for driver power state failure occurred (last recorded for dumps). It's entirely possible it was fixed from that update, otherwise I did attempt a clean install with DDU back then (around January) when I saw similar crashes back then, but obviously none of that changed anything leading up to this thread.

I didn't know about the video engine timeout error however, could that possibly be linked to the black windows I see specifically for games? I haven't seen anything yet, but as I have mentioned before, the issue does not consistently pop up -- I merely know when that particular blue screen is going to occur if games refuse to launch and I look to restart/shutdown.

I am still on the loaner CPU, but I don't think that could contribute to the problem, could it?
