How can I preserve the author identity in Track Changes in Word?

Whilst making the changes it distinguishes between different authors, but when the document is saved the original author identity is lost; the identity is shown as 'Author' for every change.

If it's happening with a .doc, then do definitely check whether "Remove personal information from file properties on save" is checked at File | Options | Trust Center | Trust Center Settings | Privacy Options. This is a per-document setting, available only for Word 97-2003 format (*.doc) files, and if these are files you did not create, then that could well be the explanation. I have no explanation for why it would be happening in a .docx file.

Edit (1/2/17): Obviously, in this very old thread, we were feeling our way to a solution. What I know now that I didn't when I posted this is that the "Remove personal information from file properties on save" box is checked for .docx files when you use the Document Inspector to "scrub" the document. That check box is not available to be checked by the user for .docx files, but when it is checked (through use of the Document Inspector), it can be cleared by the user.
