Xbox one controller randomly firing when playing COD

Nothing but headaches here.  My first console had the controller disconnecting issue, so I took it back to the store where I purchased it.

My second console/controller worked fine the first day, but on day #2 weird issues started popping up.  When I press left trigger in COD Ghosts to pull up my sights, it will pull up my sights and fire simultaneously.  It also happens when I push the B button.    Oh, and the disconnect problem is back.

I suspect the disconnect problem has to do with my console being located about 15-20 feet from where I play in a media closet (I keep the door open).  What a crappy situation.  My xbox 360 controllers never had such problems, and the 360 is located in the same media closet .   

I thought maybe my "random shooting" problem may have been due to the controller being too far from the console, but I got up, walked right up to the media closet and played and it did the same thing even with the controller 1 foot from the console. 

I guess I am going to buy a 2nd controller and see if it does the same thing.  Or maybe I will take the whole thing back and wait till they get all this crap fixed and buy the console at a later date.

My controller is randomly looking back and setting GPS points on NFS :/
