Game like "Medal of Honor" (and any other game made by EA) has a thing call an "Online Pass". It's a code you'll receive in order to access the online content for that game. When you buy the
game brand new, that code will be free. If you buy the game second hand or rent the game you will have to purchase the code for 800Microsoft Points (about $10), because it is a single use code.
This "Online Pass" is separate from Xbox Live, and is something the publisher of the game implements themselves.

No, you need a gold subscription to play online.
