Xbox Live Gold Membership what have in adittion?

Game like "Medal of Honor" (and any other game made by EA) has a thing call an "Online Pass".  It's a code you'll receive in order to access the online content for that game.  When you buy the game brand new, that code will be free.  If you buy the game second hand or rent the game you will have to purchase the code for 800Microsoft Points (about $10), because it is a single use code.

This "Online Pass" is separate from Xbox Live, and is something the publisher of the game implements themselves.

No, you need a gold subscription to play online. This link will show you the difference between account tiers.

You'll need Xbox Live gold in order to play online for every game.  That "Online Pass" is only for that one game.  If you buy Medal of Honor, and play it on a silver (free) account, you will still be able to access the single player campaign, but not the multiplayer component.

Be aware there's a few games out there that use an online pass system these days so it's worth doing your research on a game before buying.
