Black screen when trying quitting a game installed on hard drive only when CD is in tray

I've had this issue on my 360 slim ever since I bought the console and wondered if anyone else has had the same issue and knows if there is a fix. Its not been a major issue as I can get around the problem and dont fancy the hassle of packing it up and sending it back to Microsoft.

Basically if I have a CD in the tray and I load up a game I have installed on my hard drive this works fine. The problem I have is when I exit the game to go back to dashboard the screen goes black and the CD tray makes a noise like its trying to read the disc. The only way I can get around this is to make sure I have taken the disc out before playing a game on my hard drive. Unfortunately this also happens when I play games I have installed from the CD to my hard drive like Fallout: New Vegas. I have to open the disc tray to force it to quit back to dashboard.

This doesn't happen if I am playing a game straight off the CD.

When I bought the slim I transferred all the data from my old hard drive from my old XBox and I havent cleared the cache in years so I may try this but I wondered if anyone else had this issue?

Clearing the cache is the first thing to try when you have freezing issues with the console.
So I would give that a go first EXODAN.

Go to memory> highlight the HDD> press Y> select yes.

I cleared the cache yesterday, restarted my XBox and it all seemed ok, my problem had gone. However today after playing Batman: Arkham Asylum I quit the game to go on Video Kinect to chat to a friend and had to do a 52MB update first. When we finished chatting I exited Video Kinect (still with Batman in the CD tray) and the problem started again.

Have you tried a different storage device (HDD or USB) to install the game onto? Perhaps the HDD is the cause.

here's what i've found.. after quitting the game, instead of returning to the dashboard I hear a sound like the xbox is trying to read the game cd, if i do nothing then it will continue for about 3 or 4 minutes and eventually give up and return me to the dashboard.

Instead of doing nothing - if i eject the cd then it will immediately return me to the dashboard and be ok.

I tried deleting the cache as suggested but found no difference. so i removed the hard drive, and just played the game from cd, and it fixed the problem immediately. then i reattached the hard drive and the issue returned immediately. so i deleted the installed game and tried just playing it from the game cd but with the hard drive attached, that worked perfectly and the original issue didn't return. so then i reinstalled the game onto the hard drive and the problem returned immediately.

so... now i'm pretty sure its the hard drive causing the problem, this happens with every hard drive installed game i have by the way..

so i'm thinking my next step will be to try reformatting the hard drive to see if that would help.. i have to get a usb stick to copy my data first though..

although maybe I will remove the hard drive and install a game on a usb stick and try that first - I need to get a bigger usb stick, can it be any brand of usb? or does it have to be microsoft?

let me know what you think, anything else I could try? anything I should be aware of before reformatting? will reformatting affect my licenses?

I already tried this, didnt work. Not installing the game to the hard drive because of this is no big issue. The main problem I've found is when you have a game in the tray and you go on Video Kinect or play a game off the hard drive.
