BSOD on Boot (STOP 0xF4)

Quick History:
- Win 7 SP1 with all patches. Only a few months old.
- Installed on a RAID array with Intel RST with 3 x Samsung 840 EVO SSDs. There's one RAID 5 partition for the system, and one RAID 0 partition for the Page File.

PC has been working excellently for a few months (it was only just installed). Yesterday one of the SSDs came un-synced from the array causing the RAID 5 to go into recovery mode, and the RAID 0 to fail. I attempted to rebuild the array to the SSD. It started the rebuild OK, but failed at some point.

I powered down the PC, removed the SSD and read the SMART values in another PC. No errors were logged in SMART.

I re-installed the drive, tried to boot windows, and I just get an F4 BSOD during the boot sequence. I can't boot in Safe Mode as it just restarts at a similar position in the boot process.

In case this has something to do with the Page Drive, I booted from different drives, rebuilt the RAID 5 array using the same SSD. It rebuilt correctly. I recreated the RAID 0 array, formatted it, etc.

I've swapped out the SSD SATA Cable.
I've run SFC with /OFFBOOT and /OFFWINDIR (CBS Log File: http://sdrv.ms/19YidGL).
I've run Chkdsk.
I've disabled the page file on the RAID 0 array in the registry.
I've attempted to boot without driver signing.
I've attempted to boot last known good config.
I've attempted to run the automatic boot repair tool.

I'm running out of ideas.

Can someone take a look at this dump file that gets created to see if there's any clues as to what's wrong?

Intel RST is terrible. As far as I know, having verification issues is common among Intel RST + SSD users. I find it quite interesting that chkdsk is showing bad clusters, that's no good.

Is the firmware up to date on the SSD?
