Are you using an official MS HDD?.

Try clearing your cache.
Go to memory> highlight the HDD> press Y.
This will perform maintenance on your storage device.

Then try to re-install the game and see if the problem persists.

I have the same problem with black ops, MW2 and homefront on my slimline, i had to delete from hard drive and clear the system cache before reinstalling. I done this with black ops 3 times homefront 2 times and MW2 2 times not sure why, it just does it out of the blue hope this helps.

The same happened with me but with COD black ops!

I think that the memory in the hard drive is not enough. My hard drive is about like 6.6 GB I think and I installed Black ops and it says "Insert game disc" soy its because its not enaugh memory i Think
