Audio issue when switch from game (COD Ghost) back to TV.

When I switch back to TV after playing a game audio sounds like everyone is sucking on Helium.  This doesn't happen all the time but enough that it is becoming annoying.  My current fix is to restart the xbox one console.  Again, doesn't happen every time.

I am sorry to hear about the issue. To get a better idea of what is at play here, the following information is very useful.

1. Make & Model of STB (cable/sat box)

2. TV Provider

3. Layout (example: STB HDMI Xbox One HDMI TV) (please note splitters and hubs).

I am not going to have you jump through 1000 hoops if I can avoid it. But, the info above helps a lot, there may be a simple setting issue.

I am using a DISH NETWORK JOEY which is direct HDMI connect to XBOX and XBOX is direct HDMI connect to TV.  I also have the audio optical connection from XBOX to my Sony STR 997E reciever.  The audio is garbled (Helium Voices) in both stereo reciever output and TV output.  Happened again last night.
