Anyone ever send a game in for replacement

I bought the Hall of Fame edition for my husband, I actually ordering a little while before release.  I gave it to him today and upon opening it there were a few problems.  The game was missing the online pass that it was suppose to come with and the autographed Faulk card was damages pretty bad.  I do not collect the cards myself but my husband collects them and just from that experience I can say the card is not worth much in this shape.  I contacted EA and the 2 reps I talked with said to return the complete game for exchange.  Problem was they kept referencing a damaged disc.  I told them a few times the game itself was ok and the case was fine but the code was missing and the card was damaged.  So, has anyone ever sent anything like this back to EA for replacement?  And if you did, was the item they returned to you complete?

I haven't but as long as you have proof of purchase and you are exchanging with EA,  I don't think it should be a problem.
