I have tried my debit card, credit card, and paypal account, all three say the exact same thing.  I have also checked with my banks and confirmed they are not doing anything to block this transaction.  This leaves me to believe that something on my XBOX Live account is messed up.  We used to be able to add points until sometime in late November.

Verify in your payment information if they are all correct according to your billing info from your bank.

In your profile page live.xbox.com/.../account click Billing Contact Information in the Payment  & Billing section. If there´s any information in there that don´t match your bank statement correct it and try again.
