Two Xbox Live Gold subscriptions (monthly & yearly) running simultaneously on my account

First question: Why has that been possible ? : Two Xbox Live Gold subscriptions (monthly & yearly) running simultaneously on my account.

Second question: How do I get the money back for the monthly fee that I paid during my yearly subscription?
