Wasn't a PNY USB stick by any chance is it?

-

All USB drives are not created equal and some have problems working with the 360 even if they pass the configuration test, with the results you've discovered.

-

This generally occurs if you leave the stick plugged in when you turn off and on your 360. The way to stop this happening would be to sign out of your account (guide button and then X) and then remove the USB before powering off. Then plug it back in only when the console is back on.

-

You'll be able to recover your profile from LIVE and download any DLC that was on the drive but anything else that was on there, such as saves, is gone for good I'm afraid.

I have the same problem, but I had a bunch of stuff from a license transfer on it from my old account, is there any way to get that stuff back? (But for some reason castle crashes and all my DLCs still work)
