Sorry you're having issues with your headset. First try the adapter in another Xbox controller to make sure there is no issues with the controller itself. Do you have another headset or the adapter to use the chat headset to test the adapter with?
If another headset works then the issue is with the stereo headset and you may need to purchase another. If no headset work with the adapter, then it's likely a faulty adapter. If you've had this product less than 90 days you can request a warranty replacement.
If it's over the 90 days you'll unfortunately need to purchase another one.
