Have you tried using a different flash drive? And what brand is this flash drive you are using currently?

From reading this forum this seems to be a problem with PNY drives. The solution seems to be to not plug it in until the console is at the dashboard and to remove it before powering off.

This means you will have to Sign out of your profile before removing the USB as it will cause corruption to happen.
