Is it just this game that seems to be exhibiting problem, or is it more widespread?

As you've replaced everything else, this may be a fault with the network port on the Xbox. A good way of testing this would be to stream something to it from another device on your network. If this shows the same problem then it is likely to be your Xbox at fault. You can request a repair at support.xbox.com/.../repair-process.

Does this happen with just NHL12? Have you tried using a different Ethernet cord?

Not to be rude but netgear is the worst router ever. It will cause you to disconnect a lot. Use a different brand.
