I got this same error. Only happens with Xbox One games and not blurays.

Error 0x21 Your disc may be dirty or damaged. Try cleaning it, or choose a different disc.

Will contact xbox support over the phone and post here.

I am in the Xbox Preview group and I had this issue whilst having my Xbox One in "instan-on" mode when I received the update today. Must have happened in the last two hrs (GMT+1).

The solution is as AEQUITAS719 said:

  • hard reset your Xbox One (hold power button >8 secs. until it turns off, you can hear the harddrive powering down when it is very, very silent around you)
  • Afterwards turn it back on and be patient. Past the green logo it took the console about 1½ minute until the black screen with the white XBOX logo showed up
  • *Enjoy your Xbox One ;)

Yes preview update is the issue and its software related. Instant on mode.

The best way to power cycle

1) unplug the power cable from the back of the xbox one

2)unplug the power cable going into the power brick

3) wait till the orange led light on the power brick to switch off

4) connect it all back, switch it on and it should have fixed it.

Customer Services at xbox are the best ive ever come accross.
