It seems to only happen with Titanfall. I can't recall if it happened with Forza 5, maybe once or twice. Almost every time I turn on my xbox one and select Titanfall (from the disc tile on the home page), it says that I need to visit the store, cancel, or help. The disc is in perfect condition, and I do not get an error code. So every time I want to play, I have to go to the console, eject the disc, then put it back in to play.
