What happened to my start menu?

When I clicked on my Start menu, normally I would see the multiple groups and apps I pinned to Start. Such as calculator, notepad, etc. However, everything was gone. It looked like it resetted all of the apps I had pinned. I could see the default apps pinned and the Start settings reset (e.g. more tiles on Start was disabled.) Is this a new glitch?

I don't know when this happened, but from my past activity it could be one of the following:

-Restarted Windows Explorer

-Signing out of Windows

-Installing a new program (f.lux)

I think it might've been restarting Windows Explorer, since it wasn't responding.

I repinned all of my apps back again, although I don't want this to happen again.

Another thing I noticed was every app said "New"(some don't since I reopened/pinned them on Start again.)

Edit:

Any apps on my desktop I try to pin to Start, it doesn't appear anywhere as a tile but the Recently Added column adds it as a "new" app.
