Windows Defender Problem (Threat found, but not shown in Protection history)

Recently, when I scan my device using Windows Defender, it shows "1 threats found" and action has been taken. However, when I click into "Protection history", rhere is no recent action by Windows Defender, eventhough in "Allowed threats". This problem happens every time I turn on my device, the first time scan will detect 1 threat, and no more threat after first scan. I also have done with the Microsoft Safety Scanner to scan my device, but still there is no threat found. 

The followings are the Windows Defender information:

Try running these programs:
MBAM free: https://www.malwarebytes.com/mwb-download/
Eset online scanner: http://www.eset.com/us/online-scanner/
Adwcleaner: https://www.malwarebytes.com/adwcleaner/

If these find one or more infections but do not fully remove them it will be wise to register with a malware removal site to receive dedicated malware removal instructions, an expert will remain with you throughout the process until confirmation that your PC is 100% clean.

If these show you’re clean then do this:
Open the C folder & select show hidden devices under View, open ProgramData>Microsoft>Windows Defender>Scans>History>Service & delete all the files inside. Then scan with Defender again.
I’ve found sometimes Defender gets ‘confused’ & shows an old/remedied threat as new. Often this threat was found on an external drive which isn’t connected.

I'm seeing the exact same issue on my laptop when restarting and doing a scan, noticed this threat showing up since start of this month or so somewhere i think

Could this be some sort of bug or some actual malware? Nothing show's up in protection history for me either, after the system claims the action has been resolved...
