Update the display drivers and check if it helps. To do so, follow the steps below:
1. Press “Windows + X” and select Device manger.
2. Select Display Adapter and expand the driver icon.
3. Right click on the driver icon and click on Update.
4. Follow the onscreen instructions to update the driver.
If issue persist, go ahead and create a new user account and check if that helps resolve the issue.

There isn't a make or model as I built it myself.
I tried what you suggested before writing the question and it didn't do anything. I have 3 Microsoft Accounts and I tried all three of them but the issue still happens. It keeps happening ever since Windows 10 was released, I've had enough of it now and I'm thinking of reverting back to Windows 8.1. It also happens with Action Centre.
I'm using Nvidia Graphic Cards and I'm using the latest driver 359.06 which was released on 1st December 2015. All drivers are up to date.
