Hello Avi,
Thank you for posting your query on Microsoft Community Forum.
As per the description, I understand that your USB flash drive converted into RAW file system, and you are unable to change the file system, initialize or format it.
- Have you tried connecting the flash drive to any other USB port? Do you get the same result?
- Have you tried formatting the flash drive through any other computer? Do you get the same error?
- Are other USB devices working fine on the computer?
Let’s try these methods to check if that helps.
Method 1:
Let’s first run Hardware and Device Troubleshooter, and check converting the drive again.
http://windows.microsoft.com/en-in/windows7/open-the-hardware-and-devices-troubleshooter
Method 2:
We can try converting the file to NTFS through Command prompt.
Convert a hard disk or partition to NTFS format.
http://windows.microsoft.com/en-in/windows7/convert-a-hard-disk-or-partition-to-ntfs-format
Important: Converting a hard disk partition or volume from NTFS to FAT32 requires that you reformat the partition, which deletes any data on it. Be sure to back up any data you want to keep before you begin.
Method 3:
Format drive through DiskPart tool in command prompt.
- Open an elevated command prompt as suggested above.
- In the command prompt, type diskpart and press Enter.
- Next type list volume and press Enter. (This will let you see what the drive letter is for the drive that you want to convert/Format).
- After making note of the drive letter, type “select volume <x>” and press Enter, where <x> is the volume you wish to focus on.
- Type “clean” and press Enter.
- Type “format fs=ntfs quick label=test” and press Enter. (This will now perform a quick format, using a ntfs file system and labelling the drive “test”. You may wish to use the file system fat32 in place of ntfs if you would like to use the drive with other operating systems).
- You can close the Disk Part tool and Command Prompt using the “exit” command.
Hope this helps, please reply for any further assistance.
Thank you

Thanks for your suggestion. I already try with above first three solution plug USB in another port , plug USB in another computer and any other USB working properly in my commuter but problem remain with my USB.
I apply the method 1 "Hardware and Device Troubleshooter" but I don't get back my USB.
I post the log of troubleshooting:
I don't able to apply method 2 and method 3 because of USB not shown in "My computer" while USB shown in windows disk management as a not initialized disk.
