Using Outlook for Electronic Medical Records and Scheduling

Can anyone suggest how I can use the features of Microsoft Outlook 2003 to schedule patients using the Calendar function?  I'd like to be able to click on the patient's name in the scheduler then have a window pop up which contains a "medical record."  This medical record should have dates of previous "meetings" with the "contact" (patient), and "notes" (charting" from past visits.

I've played around but can't figure out a way to do this.  Any suggestions would be really appreciated!

It works fine for us, but that's because we developed it ourselves--one of the advantages of being the largest cardiology practice in the US. I think trying to cobble an EMR out of Office programs would be nearly impossible.
