Creating Forms in Word

Good morning all

Quick question - hopefully. When creating check boxes etc on forms in Word, is there any way to keep the headings you give tick boxes etc? I have created a form with several of these but until you click onto the check box, you don't know what the question is because the text doesn't stay?

The best way to create forms is to put everything in a table. Then everything stays in place. If needed, a table cell can expand vertically to hold more information, but that doesn't mess up the rest of the form:
