Xbox permanently stuck in 50hz

Following the recent March update the 50hz option for TV viewing while auto defaulting back to 60hz for other services was working great for a week.  Now without warning everything remains at 50hz.  The old workaround to switch between 50/60 doesn't even work and everything I do whether TV or games runs at 50hz.  Even if I disable the 50hz option in the settings the only frequency I can run is 50hz?  When changing resolution it will flip to 60hz for the 15 second warning but whether I accept or cancel it always flips back to 50hz.

This is really frustrating after believing the problem was addressed in the update.  I've tried a full power off, resetting the tv settings but I'm stuck in 50hz.

While I need this for TV (as that stuck in 60hz would be more annoying), I do have to suffer minor quality issues in my games.

Is this a known issue?  Any advise to resolve?  I'm assuming a bug in the software?

Had to flush the cache (hold power for 10 seconds and wait for the white/orange power brick recycle)to get it working again.  Not ideal but it is working at least.
