Answer

My installations that reportedly failed and are now showing up in wsus as not failed but installed. (not needed anymore)  the failed error is cleared.  Also, after unaproving the update yesterday if i right click and go to revision history, it looks like revision 101 has been released.  can someone confirm that is the revision that fixed this issue?

Thanks to responses on this listserver, we've looked into the Silverlight update, have identified a metadata (logic) error and will be releasing a revision to correct this very soon.

Consider holding off on this update until a revision is published and visible in your WSUS/SCCM consoles - or simply allow auto-approval to permit the revision to replace the original version.  We have similarly 'down throttled' unmanaged (consumer) machines until the revision is released (throttled at 0% = no consumer machines will get this installed automatically - but users who open WUApp and choose it, will still be able to run it - even though it will fail).

I have tried manually installing the update from the KB Article and I receive a message that the software is already installed.  I'm not sure why it would pick it up as a necessary update if it was already installed.

Looks like this is happening to all types of clients not just x64 clients.  If you go into programs and features in control panel, it's listed as installed with the correct version and install date of 2/14. so it looks like it might have installed even though it's reporting incorrectly.  odd.
