Several of us experienced this issue as well. In the error log, you would see a 1642 error. It doesn't seem to matter if you're using Windows 7 or Vista, 32 bit or 64 bit. What does seem to matter is if you already have the KB2251481 installed that was released in June. The KB2251481 from August 9 tries to install and crashes because the one from June is already installed. Seems to us like a detection problem that MS needs to fix.
Anyway, my co-worker found a workaround. Go to Add Remove Programs and uninstall the KB2251481 patch that was installed back in June. Then go to Windows Update and install KB2251481 from August 9. That worked for us on 3 different machines.

2. What is the exact error? Are you referring to error is "66A?”

If the issue still exists, you may download and install the update in a clean boot state.
After installing the update you may put the computer to normal mode as mentioned in the step 7 in the article.

I have the same problem.
It seems that the Microsoft Visual Studio 2005 Premier Partner Edition which is installed by the SQL Server 2005 installation is causing the problem. No idea to fixing it yet. Here is part of the log from the update.

x86 OS instead of x64.
SQL 2005, 2008 and 2008R2
VS 2005 and 2010, and 2010 express as part of XNA

All software updated regullary via Windows Update, no other pending updates except KB2251481 that fails with same error code 66A
I have tried both suggested solutions, with Windows Update troubleshooter and a clean boot followed by a manual install, but no luck whatsoever.
During manual install of the downloaded patch (KB2251481) it says that the target product is not installed on the system. I definitely have it.
When I look into C:\Program Files (x86)\Microsoft Visual Studio 8\Common7\IDE\Xml I can see the following file:
Microsoft.XmlEditor.dll, ver. 2.0.50727.5065, size 741,376, date 19-Nov-2010
which is the same with the one stated into the file list of KB2251481 from here:

Not sure at all if this is the file version that has to be updated or is the updated file already.
If the file is already updated then why the update process can't see this?

I am trying to run the update for Security Update for Microsoft Visual Studio 2005 Service Pack 1 XML Editor - KB2251481.
I keep getting a "66A" error.

The message states that "some" updates" were not installed. I am only trying to install the one update by itself.
I am running MS Vista Home Premium.
I am assuming that the steps you listed to try to resolve this are the same for Vista as for Win 7.
I read a possible solution on a Microsoft forum. It said that I should go to the Control Panel - Uninstall window. I should right-click on the Microsoft .Net Framework 4 Client Profile and repair/reinstall it. I did that successfully.
I tried running the update again; and, I still got the 66A error.

John's solution worked for me ... THANK YOU!!! That is, go to Add Remove Programs, click on View installed updates, and then uninstall the KB2251481 patch that was installed back in June ... this is down near the bottom under Microsoft Visual Studio 2005 (and not under Microsoft .NET Framework per another suggestion). Then, from Windows Update, retry the new KB2251481 again. Both steps took 30-45 minutes (on my rather old laptop), but no restarts required.
I am running Windows 7 Enterprise. I had attempted "Repair the .NET Framework installation", the "Windows Update Troubleshooter", and "Microsoft Fix it" to no avail ... but I mention this on the very remote chance one of these might be needed before John's fix. I did not attempt the Clean Boot, so this should not be needed.
I can confirm John's solution is working allright. Just uninstalled old KB and applied the new update.
What is strange is that I can't see any change for Microsoft.XmlEditor.dll that MS says it's updated with this pack. The file version / size is the same for the June update and for the August one.
I think this is something Microsoft messed up this time (again).
