Errors in Access Label Wizard

All of a sudden there is only one available field in the ACCESS label wizard {ID}  the other fields have vanished.  I tried repairing OFFICE but no changes.  How can I retrive or add the fileds choices that used to be there {First Name} {Last Name} {Address} {City} {ST} [ZIP}?

The label wizard will only show the fields that are defined in the table or query in which you are trying to create labels off of.  What table or query are you trying to create labels off of?  If you open that table or query in design view, how many fields does it have? 

 

As another test to ensure the wizard is working, what happens if you download the Northwind sample database and then within this database, select the Employees table from the navigation pane and then click on the Create tab in the ribbon and click the Labels option?
